In order to process financial aid, there are times we are required to request additional documentation to support and verify the information. If you are required to submit documentation, you will be notified by your university email. Open requirements will also be listed in your Financial Aid Self-Service Portal. At this time, we encourage all students to explore online options for conducting business.
With FA Student Forms process you will be able to:
- Complete and upload financial aid forms on your phone, tablet, or computer
- E-sign documents
- Receive automated reminders about outstanding tasks and next steps
Note: The first time using this platform students will be asked to complete a one-time account creation. Students will be asked for some basic information to validate their identity so that electronic signatures can be used on future forms.

FA Student Forms FAQ
Students will access FA Student Forms using their UMICH login, the same login credentials as majority of other university platforms. The first time logging in you will be asked to complete a one-time account creation to validate your identity so that electronic signature can be used on future forms.
Make sure you enter the Social Security Number in this format ###-##-#### with the hyphens (-). The Date of Birth and Names must match exactly how you report it on the FAFSA.
You have the option to download a copy of the Verification Form for signatures and upload the signed form to Student Forms. You must click to cancel the signature request first, then click to “Fill Out” the Verification Form again, navigate to the Review & Sign section, scroll down to click the “Opt-Out” of e-sign, then click “Download” for a copy of the completed form for you and your parent to sign and upload. Please note that both the student and parent must submit the signature in the same fashion: either both e-signing or both downloading/uploading.
Make sure that you have canceled the signature request before you can make changes to your verification form.
You must e-sign the form again for the changes to take effect. If you opt-out of e-signature and choose to download the form instead, then you must click “Download” when you are at the Review & Sign section of the form for the changes to take effect.
