Required COVID Vaccination

  • All University of Michigan students, faculty and staff are required to receive the COVID vaccine including the booster shot, with limited exceptions. This policy is effective immediately and subject to change as pandemic conditions evolve. For employees covered by a collective bargaining agreement, the provisions of the collective bargaining agreement shall apply. This policy does not apply to third party vendors, patients, guests, and visitors.  
    • Faculty, staff, and students are required to be fully vaccinated for COVID-19, which includes receiving all series of a vaccine and any additional boosters, by February 4, or when eligible.
  • Fully online/remote students will be exempt from the vaccination mandate with a signed attestation form. For more information visit the Vaccine Mandate Enforcement page.
  • If your COVID-19 vaccination information has already been submitted and verified, you do not need to take any further action.  
  • Individuals must have completed, or received at least the initial dose of, the COVID-19 vaccination and submit their proof of vaccination information to U-M no later than January 3, 2022 via the forms below:
    • To report that you have received all doses of a vaccine series, submit information via the COVID-19 Vaccination Completed form
      • To be considered fully vaccinated and compliant with the vaccination policy, booster shots will be required for all faculty, staff, and students by February 4, or as soon as eligible thereafter. This will provide an added layer of protection for our campus community. Booster shots are available free of charge at the clinic in the University Center.
    • To report that you have received one dose of a two-dose vaccine series, submit information via the Partial COVID-19 Vaccination form.
    • To report your booster information, submit information using the booster form.
    • Contact the Campus COVID-19 Call Center if you need additional assistance submitting your information. The Call Center can be reached at 734-936-7000, Monday-Saturday 9 a.m.-10 p.m. and Sunday 9 a.m.-8 p.m.
  • The university will grant limited exemptions for valid medical and religious reasons. To be considered for a vaccination exemption, please complete the COVID-19 Vaccination Exemption Request Form. Those who are granted exemptions will be required to submit a weekly negative COVID test before being on campus.
    • If you have an approved vaccination exemption you do not need to submit a booster vaccination request.
    • If you are fully vaccinated and would like to submit a booster shot exemption request, you may use the Booster Shot Exemption Form to submit your request for consideration.
  • You are required to get vaccinated even if you've already had COVID-19. Even if you have already recovered from COVID-19, it is possible that you could become infected with the virus that causes COVID-19 again. Studies have shown that vaccination provides a strong boost in protection in people who have recovered from COVID-19.
  • Advisers for faculty and staff will be available by phone (734-764-8021, prompt 2) weekdays from 8 a.m. - 4:30 p.m. Students who have questions about the vaccine are invited to sign up for a Zoom discussion with a clinician or nurse using the University Health Service website weekdays from 8-10 a.m. Advisors will not engage in conversation about the policy.
  • If you lost your vaccination card - students, faculty and staff can access their personnel vaccine records by going to Once on the MCIR site select the “public tab” on the left-hand side of the page. Set up an account and upload a government-approved document with a photo. Follow the prompts.
