    I (Incomplete) Contract Form -  May be submitted in person, by mail or by fax to the Office of the Registrar.

  • Name Change

    Name Change - May be submitted in person, by mail or by fax to the Office of the Registrar. Official documentation must be attached.

  • Non-Disclosure

    Non-Disclosure - Must be submitted in person or by mail to the Office of the Registrar.

  • Registration, Add & Drop Form

    Registration, Add & Drop Form - Must be submitted in person to the Office of the Registrar with appropriate signatures. Please refer to the Academic Calendar for deadlines and impact on fees for adding and dropping.

  • Request to Participate in Commencement Ceremony

    Request to Participate in Commencement Ceremony - August graduates only. May be submitted in person, by mail or by fax. Please refer to the Commencement page for deadline dates.
