In order to process financial aid, there are times we are required to request additional documentation to support and verify information. If you are required to submit documentation, you will be notified by your university email. Open requirements will also be listed in your financial aid portal.
Effective with the 2022-2023 academic year, we are now offering a new easy, mobile, personalized process that makes everything about navigating financial aid easier. With this new FA Student Forms process you will be able to:
- Complete and upload financial aid forms on your phone, tablet, or computer
- E-sign documents—both you and your parents!
- Receive automated reminders about outstanding tasks and next steps
Note: The first time using this platform students will be asked to complete a one-time account creation. Students will be asked some basic information to validate your identity so that electronic signature can be used on future forms.
FA Student Forms FAQ
Students will access FA Student Forms using their UMICH login, the same login credentials as majority of other university platforms. The first time logging in you will be asked to complete a one-time account creation to validate your identity so that electronic signature can be used on future forms.
Make sure you enter the Social Security Number in this format ###-##-#### with the hyphens (-). The Date of Birth and Names must match exactly how you report it on the FAFSA.
You have the option to download a copy of the Verification Form for signatures and upload the signed form to Student Forms. You must click to cancel the signature request first, then click to “Fill Out” the Verification Form again, navigate to the Review & Sign section, scroll down to click the “Opt-Out” of e-sign, then click “Download” for a copy of the completed form for you and your parent to sign and upload. Please note that both the student and parent must submit the signature in the same fashion: either both e-signing or both downloading/uploading.
Make sure that you have canceled the signature request before you can make changes to your verification form.
You must e-sign the form again for the changes to take effect. If you opt-out of e-signature and choose to download the form instead, then you must click “Download” when you are at the Review & Sign section of the form for the changes to take effect.
Chrome is the recommended browser for Student Forms, but Firefox, Safari, and Edge are also supported.
University of Michigan Employee and Dependent Scholarship Applications
Employees (Ann Arbor, Dearborn, Flint, and Health System) attending courses at UM-Dearborn must complete this form each semester and submit to the University of Michigan – Dearborn Office of Financial Aid and Scholarships. For eligibility consideration, the form must be submitted prior to the start of classes. The University of Michigan – Dearborn Employee Scholarship may be used in addition to the U-M Tuition Support per SPG. 201.69 *

In addition to applying for admission and being accepted to the University of Michigan-Dearborn, the student must submit the UM System Dependent Tuition Scholarship Application along with the family’s most recent federal tax form as supporting documentation of the dependent status of the student.
To verify eligibility of a child, the family's most recent federal tax form as supporting documentation of the dependent status of the student is required.
To verify a spouse or Other Qualified Adult (OQA), the following verification is acceptable:
- verification of the spouse/OQA on the staff/faculty member's University of Michigan benefit plan through Human Resources;
- or verification through submission of the family's most recent federal tax form as supporting documentation;
- or a marriage certificate may be provide for the spouse;
- or OQA status verification certifying that your OQA qualifies as an eligible tax dependent under Section 152 of the Internal Revenue Code for the most recent tax year.
