Budget Clearance Forms:
A Budget Clearance Form is required before processing most personnel actions on the Dearborn campus. Please see this link for guidance on when Units are required to process a Budget Clearance Form.
NOTE: Effective July 1, 2022 the Budget Clearance Forms (Excel and PDF versions) were replaced with a Unit specific Budget Clearance Approval Google worksheet. Each Unit's worksheet includes directions on data entry, form definitions, and approval routing. You should contact your Unit's budget lead FIRST for assistance with accessing and completing your Unit's Budget Clearance Approval worksheet, as your Unit may have specific data entry and approval procedures to follow. If your Unit budget lead is unable to answer your question, then feel free to contact us here in Financial Services for assistance.
Cash Receipt Deposits
Cash Receipt Form
All funds collected by any unit or department must be deposited into a University
Account, according to the standard practice guide.
Cash Receipt Instructions:
- Short code: required- 6 digit numerical- can be looked up on Dearborn Chartfield
- Name: this will assist if more information is required
- Date: Date filling out the deposit
- Phone number: Enter the number at which the preparer can be reached
- Describe Nature of Receipt: require (30 characters): Enter an adequate description of the transaction. This description will appear on the Statement of Activity.
- Account code: required- 6 digit numerical, this is not your short code.
- Chartfield Combination: required -Enter the appropriate Chartfield combination that the deposit will be credited to. Making sure the Account Code, Short Code and Chartfield are compatible with each other.
- Check Amount: list each check separately, listing check number, date of check, name and amount of check. Stamp it on the back, writing the shortcode in. If you don’t have a stamp, just write your shortcode on the back of the check.
- Add the checks up and write the total at the bottom.
- Cash deposit: add cash and write the total at the bottom
